You will need to complete the top portion of the Residency Verification Form (one copy per student).
Bring proof of Tdap (Pertussis Booster) for each student.
Bring the approval letter from the Acalanes Union High School District approving your request for inter- or intra-district transfer to attend Miramonte High School for the 2011-2012 school year.
